For existing patients, you can access your patient data by clicking the patient portal button below and logging in. You may book and reschedule appointments in the patient portal as well.

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Request Medical Records
Your medical record includes your health history, allergies, medicines, test results, and how to contact you. So, it’s our job to protect your privacy while giving you timely access when you need it.
Records are stored electronically. The state of Georgia regulates how much we can charge per page to cover the costs of labor, time, and resources needed to prepare your request. We comply with state and federal regulations related to the release of medical record information.

Insurance and Payment Information
Insurance based medicine is geared towards dictating providers on what type of care they can provide. It does not allow your doctor to utilize their knowledge to provide you the best care possible. For this reason, we’ve adopted a hybrid-model billing practice.
If we are out-of-network or you have an unmet deductible with your insurance carrier, you will pay for the office visit at the time of your appointment. Unlike other offices, we are completely transparent with our billing upfront, and unfortunately insurance verification and checking eligibility and benefits is a timely process, so we will not be able to notify you prior to your appointment. It is your responsibility as a patient to find out whether we are in network or not and to know your insurance coverage.
Disability Paperwork
Sava Podiatry does not participate in completing disability paperwork. You must visit your primary care doctor for this service. If needed, we can provide your or your primary care physician with your medical records (a small fee may apply). Please complete this form, and send it to us.
